Form For Thursday 28th Shoot

Here are the forms for the shoot that I made for the shoot that we wanted to do for Thursday the 28th of January. This shoot was to shoot the footage of our second episode of Detention Deficit. The first image is of the permission letter that I wrote for using the equipment and the last two images are of the risk assessment that we needed to have for the shoot.
