Production Forms For Our First Shoot - First Episode

Here are images of the forms that I created for the shoot of the first episode. I needed to produce a risk assessment of the shoot so the shoot would be as safe as possible. I also had to produced a permission letter so that we were able to use all of the equipment that we needed for the shoot. We also need model release forms for each of the actors in the shoot, however they still need to be given back by actors who have signed them and put in our folder.
