Production Forms For Shoot on January 13th

Here are some images of the forms we needed to produce the sound for our trailer. We wanted to make a short trailer for our first episode and we had managed to get the footage but the sounded need to be completed correctly. In order; permission slip and the last two are images of the risk assessment.
