Production Forms For Shoot on January 12th

Here are some images of the forms that we needed for one of our first shoots, which was to make the trailer footage. We wanted to produce a trailer to advertise our series and I needed to produce a permission slip so we can use the equipment and also a risk assessment so that our shoot is perfectly safe.
